Diane Farraher-Smith, MSN/MBA, RN, is the Executive Director of Circle Home, Inc. as of November 2017. With over 40 years' experience in home health care, hospice and community care programs, Diane is also the President of Hallmark Health Visiting Nurse Association & Hospice, Inc., and the Vice President of Home Care and Community Programs at Hallmark Health System, Inc. She has demonstrated a passion to create and advance integrated health care that is patient and family centric. She has been honored with numerous community awards and recognitions, and serves on many boards of civic organizations. A nursing graduate of Children's Hospital School of Nursing, she received her Bachelor and Master of Science in Nursing degrees, and her Master of Business Administration degree, from Salem State College.
Director of Homecare and Hospice
Elaine Bussey joined Circle Home as Director of Clinical Services in 2016. She has over 15 years experience leading home health care and hospice services for agencies in New England. She has been an active volunteer Board member for several home care associations and community service organizations. Elaine graduated from the Mt. Auburn School of Nursing, and earned her Bachelor's degree in Management from Lesley University and her Master's degree in Health Care Administration from New England College.
Director of Finance
Eileen Trefonides has served as Director of Administrative Services for Circle Home since 2015. She has held leadership positions in multiple areas of homecare and community services. She received her Bachelor's degree in accounting and Associate's degree in management from Bentley University and received her Certificate in Public Accounting from the Massachusetts Board of Public Accountancy.
Director of Quality and Performance
Cathy Hutchins joined Circle Home as Director of Quality and Performance Improvement in 2016. She has over 13 years experience as a clinical and quality management leader for home health care agencies in Massachusetts. She has been recognized with The DAISY Award, given to extraordinary nurses. Cathy graduated from Northern Essex Community College with an Associate Degree in Nursing, and earned her Bachelors Degree in Business Administration from University of Massachusetts-Lowell.
Director of Information Technology Services
Jon Mitchell has served as the Director of Information Technology for Circle Home since May 2012. He has over 18 years of IT experience in healthcare and genetic research laboratory settings including IT leadership in the medical environment. Jon has an active history of volunteerism in the community. He has a Bachelor's of Science degree in Management.
Director of Business & Program Development
Eileen Malloy has served as Director of Business & Program Development for Circle Home since March 2015. She has been a Circle Home employee for over 30 years, most recently as a Hospital Liaison Nurse. Eileen received her Bachelor's degree in nursing from the University of Massachusetts Lowell, and earned a Certificate in Home Care Management from Suffolk University.
Director of Community Relations & Development
Irene Egan has served as Director of Community Relations & Development for Circle Home since December 1998. She has over 25 years of experience in non-profit communications, public relations, marketing, publications, development and special events. She has been recognized with several awards for her community work in Lowell. She received her Bachelor's degree in journalism from Marquette University.
Administrator, Commonwealth Nursing Services
Angela Callahan became the Administrator of Commonwealth Nursing Services in 2011. She has over 20 years of nursing experience, including positions as a charge nurse, supervisor, case manager, admissions/ marketing manager, and assistant administrator, and has served as a board member for several community organizations. Angela received her nursing degree from Rivier University and a degree in Spanish/Sociology from Suffolk University.
